I've been on leave since June 4. I've been hospitalized, intensive outpatient care, and surgery. My FMLA terminates on Aug 7, 2015. I'm currently on short term disability. I pay my monthly health premium every month. My company states that if i do not return by Aug 7, I will have to pay the company back their premium that the company pay for each employee's short and long term benefits. I'm confused, please clarify whether or not I have to repay the company

Don't agree to anything. Keep copies of everything and ask for any communication/ decision to be in writing.
Meanwhile contact your State Dept of Labor/Employment and get advice ASAP.
